All patients requesting appointment are required to answer the following questions within 24 hours prior to their appointment:

Have you had a fever in the last 3 weeks?
Have you had a cough in the last 3 weeks?
Have you had any muscle aches in the last 3 weeks?
Have you had a runny nose or lost sense of smell in the last 3 weeks?
Have you had any known exposure to someone who has tested positive, has, or has had, coronavirus?
